Associated Bank offers the ASSIST Program, which may provide an option for customers who have experienced a financial hardship and are struggling to make their payments. By working together and offering you a number of options, we may be able to develop a beneficial solution and save your home.

Possible Options:

  • Repayment Plan Agreement: This is a formal agreement between you and Associated Bank to spread out your past due amount, added on to your current mortgage payments, over several months in order to bring your mortgage current.
  • Modification: Changing the terms of your original contract in order to bring your account current and prevent the loss of your home.
  • Pre-foreclosure sale: : The sale of a property in lieu of a foreclosure, resulting in a payoff of less than the total amount owed, which was pre-approved by the servicer.
  • Deed in Lieu: This is a voluntary surrender of your property in order to satisfy your mortgage and avoid public auction of property.

Please fully complete and submit all three forms including any additional documentation required. You may print the forms and fax them to 715-345-4732. They can also be mailed to:

After we receive the fully completed forms, the information will be reviewed for one of the ASSIST Program workout options. If you are approved for one of our programs, we will need to return the 710 Form and the 4506T Form for your signature and date if not already provided.
